The staff from Transitional Home Health are a true reflection of the company and our dedication to a dignified, knowledgeable and compassionate delivery of care. We offer a range of personal care services that are provided in the comfort of your own home. Transitional Home Health will work with the client and the family member to implement a care plan that includes the client’s needs as well as the caregiver’s needs. We will deliver supportive, knowledgeable and dignified care to our clients with the objective of assisting them to remain in their homes for as long as possible. Seniors, vulnerable persons and those individuals with chronic health issues require care based upon their individual needs for a longer term, and often throughout their life. Caregivers will also require respite services to enable them to cope with these needs. We understand that the last bastion of independence lies within a person’s ability to make their own decisions. Providing the support that they and that their families need to enable this right is our priority. We are located in Sudbury, Ontario and provide services within the jurisdiction.
