F.O.C.A.S. mission is to deliver a variety of affordable healthcare services including personal support, social work, and occupational therapy services that promote wellness and self-sufficiency to enable clients who sustained an acquired brain injury and frail seniors to live independently in a safe and supportive environment. F.O.C.A.S. Personal Support Workers will: Provide you with personal care services such as bathing and getting dressed. Kindly remind you to take your medication(s). Help you to perform with light household tasks. Assist you in maintaining a safe and clean environment. Accompany you to medical appointments and social outings. Help you to perform your daily activities that you enjoy doing. Provide respite for family members who are going on vacation or require some rest from their caregiver routine. Prepare nutritious, varied and economical meals and snacks, including special diets (proper food preparation, storage and hygiene practices are always used). F.O.C.A.S Social Workers can help you and your family by: Analyzing assessment findings and identifying personal goals to develop a client-centered F.O.C.A.S. care plan. Providing counselling services to cope with the impact of illness and life stressors. Providing education and support related to psychosocial, emotional and financial needs. Advocating for funding sources and accessible services. Providing referrals to community agencies and social services. F.O.C.A.S. Occupational Therapists can help you and your family by: Conducting home safety assessments that will help clients to reduce their risk of falls and improve their independence to participate in meaningful activities. Providing strategies to ensure safe completion of their meaningful daily activities independently or with help. Recommending equipment (wheelchair) or assistive devices (long-handled shoe horn). Providing strategies to reduce falls, to transfer safely and to conserve energy. Providing information regarding community resources (Community Care Access Centre, local senior’s community services, etc.). Our initial assessments are FREE over the phone. Call Us!
